New Patient Registration

To register as a patient, you can come to the surgery to collect new patient registration forms. Alternatively these can posted out to your home address. 

We ask that you always contact your previous practice to ask for a 4 week supply of medications when you start the new registration process.

If you have a medical card, you should bring this with you.

You will be asked to provide 2 forms of identification.  One of which must have your present address.

Muiredge Surgery will be closed between 1pm and 6pm on Wednesday 1st November 2023.  if your condition is life threatening then please hang up and dial  999.  Alternatively, if you require Urgent Care between these hours, patients should call 01592 729250 for assistance.  After 6pm, please call NHS 24 on 111.  Please note that the surgery will resume usual working hours on Thursday 2nd November.
